We may also collect your health information

Personal health related information may include your NHI number, medical history, test results.

With your permission, we may collect information about you from third parties. For example: test results from your nurse, doctor (GP) or other health practitioner. 

How we may use your information

We may use your personal information for the following purposes:

  • To undertake customer service activities, such as responding to any queries, comments, or complaints you may have, or to provide you with further information as requested by you, or to update you on our work
  • With your permission, to refer you to another service provider (we may pass relevant information on to that provider as part of the referral)
  • To enable us to monitor, review, and improve our business systems, products and services
  • To comply with relevant laws and regulations
  • For any other specific purpose which we will notify you of at the time your personal information is collected, and
  • To report significant misconduct or breach of duty, or where there is a serious threat to health or safety. If our staff are threatened or abused, we may refer this to the police 

Who can access your information, and who do we share it with?

We respect and protect the privacy and confidentiality of your personal and health information. We will not share your information, or health record unless you ask us to, or we are required to do so by law.

We provide non-identifiable information to the Ministry of Health (we do not disclose personal information that can identify you).

With your permission, we may communicate with appropriate health practitioners, to ensure you receive the health services that you are entitled, such as linking you to other health providers, arranging tests if necessary, and linking you to counselling and other support services.

Research may be undertaken on non-identifiable information (we do not disclose personal information that can identify you).
